Role: Registered Nurse, Case Manager (RN)
Location: Canonsburg, PA
Position: Full Time + Benefits & Sign on Bonus

Overview:

This is an opportunity for a dedicated Registered Nurse Case Manager to join our client and make a difference in the lives of patients in hospice care. You will be part of a supportive team, providing essential care and coordination for patients and their families. 

Key Responsibilities: 

● Manage patient care in hospice settings.
● Coordinate with healthcare teams.
● Provide leadership and support to nursing staff.

Territory/Driving Expectation: 

Canonsburg and 15-25 miles +/- in a radius surrounding the community Schedule (Including any on call/weekends): 6 units daily; 30 units a week; 8 AM – 5 PM, Mon-Fri schedule. 1 weekend per month; 1 evening per week of on-call. Travel ● Local travel required covering multiple counties.
