Overview:

Our client is seeking a compassionate Home Health Registered Nurse Case Manager to join their extraordinary care team. The role involves providing management and delivery of patient care plans, supporting patients in Matamoras, PA and surrounding areas, and ensuring timely and quality patient care. 

The ideal candidate will possess an RN license in the state of work, have graduated from an approved school of practical nursing, and demonstrate clinical competence and experience in patient care delivery. 

Key Responsibilities: 

● Lead and collaborate to provide management and delivery of patient care plans.
● Support patients in Matamoras, PA and surrounding areas.
● Consistently meet critical deadlines for charting.
● Self-manage time and schedule.
● Demonstrate experience with tablets, mobile phones and EMR software.


Schedule and Shift Details: 

● Monday – Friday: 8 am – 5 pm, One weekend every 4-6 weeks, 2 holidays per year.

Travel: Local travel.
