Home Health Registered Nurse (RN)

We are seeking an experienced Home Health RN to provide skilled nursing care to adult and geriatric patients in a home health setting. The ideal candidate will have 1+ year of skilled Home Health experience, with OASIS proficiency required, and the ability to work independently while delivering high-quality, patient-centered care.

Key Qualifications & Requirements:

• Specialty: Home Health
• Experience: Minimum 1 year of skilled Home Health experience (OASIS required)
• Certification: Active BLS required
• Charting System: Epic experience preferred
• Scrubs: Any color permitted

Role Expectations:

• Perform comprehensive assessments, including Start of Care, routine visits, recertifications, and discharges
• Provide medication administration and management, wound care, and patient education
• Accurately document care using Epic and comply with all clinical and regulatory standards
• Collaborate with interdisciplinary teams to support optimal patient outcomes
