Responsibilities:

  • Supervise and mentor a team of home health nurses, certified nursing assistants , and other caregivers
  • Develop, implement, and evaluate patient care plans in collaboration with physicians and other healthcare providers.
  • Ensure compliance with healthcare regulations, standards, and protocols.
  • Conduct patient assessments, monitor vital signs, and administer medications as needed.
  • Coordinate patient admissions, transfers, and discharges efficiently.
  • Provide education and support to patients and their families regarding treatment plans and healthcare resources.
  • Participate in quality improvement initiatives to enhance patient care outcomes.
  • Maintain accurate and updated patient records and documentation.

Requirements:

  • Active Registered Nurse (RN) license in the state of practice.
  • Associate degree in Nursing (BSN preferred).
  • Prior experience in patient care management or a similar role.
  • Strong leadership, communication, and interpersonal skills.
  • Knowledge of healthcare regulations and best practices.
  • Ability to multitask, prioritize, and work effectively in a fast-paced environment.
  • Proficiency in electronic health records (EHR) systems.
