Position Summary
We are seeking a passionate Health Coach to support our obesity management program virtually. In this role, you will engage weekly with patients, serve as the primary care liaison, lead patient communication, and coordinate closely with our interdisciplinary team to ensure exceptional patient experiences. This position offers flexible scheduling, remote work, and the opportunity for growth within Luro Health. This is a virtual role that starts as part time and has the option to go full time.
As part of the role, you will receive training in obesity care management from one of the leaders in obesity care, Jamie Kane, MD. Jamie is one of the co-founders and Chief Medical Officer of Luro Health.
Key Responsibilities
- Weekly Patient Check-Ins: Conduct regular virtual check-ins with patients, tracking their progress, providing motivation, and adjusting care plans as needed.
- Patient Communication: Serve as the lead communicator, answering patient inquiries promptly, clearly, and empathetically, ensuring a seamless care experience.
- Care Liaison: Act as the primary liaison between patients and our broader care team, facilitating smooth communication and coordination.
- Patient Education & Support: Provide practical guidance on nutrition, lifestyle, and behavior modification strategies to help patients reach their health goals.
- Care Coordination: Collaborate closely with physicians, nurse practitioners, dietitians, and other team members to execute comprehensive care plans.
- Documentation: Accurately document patient interactions, updates, and care coordination notes in the electronic health record (EHR).
- Technology Utilization: Leverage telehealth platforms, technology-based patient engagement tools, and communication software to support effective remote patient management.
Qualifications
- Bachelor’s degree in Health Education, Nutrition, Psychology, Public Health, or related field (preferred).
- Certification in Health Coaching from a recognized program (strongly preferred).
- Minimum of 1 year experience as a Health Coach, Patient Navigator, or similar role (required).
- Experience working in obesity, weight management, lifestyle modification, or chronic care management (preferred).
- Telehealth or virtual patient engagement experience (preferred).
- Technical Skills: Proficiency with telehealth platforms, EHR systems, Google Workspace (Gmail, Docs, Sheets), and other digital communication tools.
- Exceptional interpersonal, communication, and motivational interviewing skills.
- Ability to manage multiple patient interactions efficiently and empathetically.
- A reliable internet connection and a private, HIPAA-compliant workspace at home.
