Position: Licensed Case Manager
Are you a licensed clinical leader who thrives in a mission-driven, hands-on environment? Rooted Life is looking for a Licensed Case Manager to help drive the delivery of high-impact, trauma-informed care to vulnerable individuals transitioning from hospital or crisis settings. This is a rare opportunity to lead a multidisciplinary care team and directly shape the quality and integrity of our Enhanced Care Management (ECM) and Housing Transition programs under California’s CalAIM initiative.

In this role, you’ll balance leadership and clinical care—supporting and supervising Lead Care Managers, maintaining a focused caseload of high-need clients, and translating policy and best practices into sustainable systems and workflows. You’ll have the chance to build staff capacity, lead documentation and compliance efforts, and elevate the overall standard of care in a rapidly evolving field.

Position Overview

The Licensed Case Manager is a senior member of the Rooted Life care team, providing both direct clinical services and day-to-day oversight of the Lead Care Manager (LCM) team. This role is central to ensuring the quality, compliance, and impact of our Enhanced Care Management (ECM) and Housing Transition services under California’s CalAIM initiative.

This position includes maintaining a small client caseload, managing staff, and overseeing documentation and service delivery standards in alignment with CalAIM requirements. The Licensed Case Manager plays a key leadership role in translating clinical best practices into practical workflows, supporting the professional development of the LCM team, and ensuring that all services are trauma-informed, person-centered, and outcomes-driven. This position is ideal for an experienced professional with experience with residential care facilities, who thrives in a roll-up-your-sleeves work setting. Key Responsibilities

Licensed Oversight & Team Leadership:

  • Provide day-to-day supervision, coaching, and oversight to all Lead Care Managers.
  • Conduct regular team huddles, performance reviews, and 1:1 check-ins with LCMs.
  • Support staff in addressing complex client cases, clinical judgment calls, and escalations.
  • Ensure documentation quality, service coordination, and client engagement are consistently aligned with organizational and CalAIM standards.
  • Address operational or client-related issues promptly, taking hands-on leadership in complex or high-priority cases.

Direct Service:

  • Maintain a small, high-need client caseload, including care planning, assessments, and coordination of internal and external services.
  • Provide clinical interventions, goal setting, and progress monitoring using trauma-informed, strengths-based approaches.
  • Serve as the primary point of contact for high-acuity cases or clients requiring licensed-level support.

Enhanced Care Management & CalAIM Compliance:

  • Maintain subject matter expert-level knowledge of ECM and Community Supports under CalAIM.
  • Train, coach, and hold staff accountable to Medi-Cal documentation standards and billing practices.
  • Assist in interpreting managed care plan requirements and adapting internal workflows accordingly.
  • Participate in audits, corrective action planning, and quality improvement initiatives.

Documentation & EHR Oversight:

  • Ensure the timely and accurate submission of documentation within our EHR system, maintaining compliance with Rooted Life’s policies and regulatory requirements.
  • Oversee and monitor workflows within the EHR to ensure all client interactions, care plans, and assessments are properly documented and up to date.
  • Review and co-sign care plans, assessments, and progress notes as required.
  • Lead documentation trainings and implement systems for improving data quality and compliance.

Process Improvement & Compliance:

  • Identify and implement process improvements to optimize service delivery.
  • Ensure compliance with all organizational policies and regulatory standards, and service requirements.
  • Collaborate with leadership on strategic projects, ensuring service delivery & capabilities align with growth initiatives.

Training, Supervision & Performance Management:

  • Oversee the development and implementation of training programs for staff, ensuring adherence to current policies and regulatory updates.
  • Provide hands-on supervision during crises, complex cases, or emergency responses

Cross-Functional Leadership:

  • Serve as a subject matter expert for behavioral health, case management, and trauma-informed care practices.
  • Identify system gaps and contribute to SOP development, automation needs, and operational improvements.
  • Collaborate with Operations, Billing & Authorizations, and Administrative divisions to ensure seamless service delivery and problem-solving.

Community Engagement & Representation:

  • Actively participate in local community meetings and events relevant to Rooted Life’s services and operations.
  • Represent Rooted Life on committees or other external groups, fostering relationships and partnerships with community organizations and stakeholders.
  • Educate the community about Rooted Life’s programs, ensuring a clear understanding of our services and advocating for our mission.
  • Build and maintain relationships with local agencies and service providers to enhance collaboration and expand Rooted Life’s presence in the community.

 

Additional Duties

  • Participate in strategic meetings, cross-site learning, and leadership development opportunities.
  • Take on additional responsibilities as needed to support team performance, client outcomes, and program growth.

 

Qualifications

  • Active, unrestricted California license as an LCSW, LMFT, LPCC, RN, RDN/RD, Licensed Psychologist or Pharmacist (required).
  • Minimum 1.5 years of post-licensure clinical experience with at least 1 year of direct supervisory experience preferred.
  • Strong knowledge of Enhanced Care Management (ECM), CalAIM services, and Medi-Cal documentation requirements.
  • Experience working with high-need populations including individuals experiencing homelessness, SUD, and co-occurring mental health conditions.
  • Proficiency with EHRs and comfort learning new technologies.
  • Excellent written and verbal communication, time management, and coaching skills.
