Position Overview 
Rooted Life is seeking a Nurse Case Manager (RN) – Lead to combine clinical nursing expertise with leadership of our care team. This role oversees Lead Care Managers, maintains a small caseload of high-need clients, and ensures compliance with Medi-Cal and CalAIM standards in Enhanced Care Management (ECM) and Housing Transition programs.

This is a unique opportunity for a mission-driven RN to provide direct client care while shaping workflows, documentation, and staff development in a rapidly evolving field.

Key Responsibilities

  • Leadership & Oversight: Supervise Lead Care Managers, provide coaching and support on client cases, and ensure alignment with standards of care.
  • Direct Nursing Care: Conduct assessments, develop care plans, and provide nursing-level interventions for high-need clients.
  • Compliance & Documentation: Oversee Medi-Cal documentation and billing practices, review/co-sign care plans, and ensure timely EHR entries.
  • Quality & Improvement: Support audits, lead process improvements, and implement corrective actions where needed.
  • Training & Development: Guide staff in trauma-informed care, compliance, and best practices; lead trainings and check-ins.
  • Cross-Functional Collaboration: Work with operations, billing, and partner providers to ensure coordinated care delivery.
  • Community Engagement: Represent Rooted Life in local networks and strengthen partnerships with community providers.

Qualifications (Must Have)

  • Active, unrestricted California RN license.
  • 2+ years nursing experience (care management, behavioral health, or acute care preferred).
  • Strong understanding of Medi-Cal/CalAIM or willingness to learn quickly.
  • Supervisory or team leadership experience.

Preferred

  • Background in behavioral health, SUD, or transitional care.
  • Familiarity with Enhanced Care Management or Housing Transition services.
