**Position Overview**
Location - Brasstown, NC
The incumbent is responsible for providing treatment and casework services to patients under the direction of the supervisor, ensuring compliance with all applicable laws,
Timely documentation of patient services based on the established plan of care is essential. The incumbent will identify additional physical, psychological, social, and spiritual needs of assigned patients and manage related expenditures in a fiscally responsible manner in accordance with the company’s budget. Travel may be required as necessary.
**Responsibilities**
1. Prepare and maintain accurate and timely patient files.
2. Coordinate care for assigned patients and effectively communicate patient status and concerns with the treatment team.
3. Conduct comprehensive patient assessments and develop and implement patient treatment plans in adherence to State OTP regulations.
4. Deliver clinical services, patient care, education, and counseling services pertaining to opiate addiction for patients receiving methadone maintenance and detoxification treatment.
5. Facilitate individual counseling sessions.
6. Provide patient advocacy and serve as a liaison with community and regulatory agencies as needed (e.g., CSAT, State Methadone Authority, Probation/Parole Office, courtesy dosing, housing, food assistance).
7. Monitor patient conditions to evaluate the effectiveness of therapy and adjust treatment plans as necessary.
8. Assist in program evaluation efforts aimed at describing and assessing service quality.
9. Comply with all Federal and State regulatory standards as stipulated.
10. File body specimen test results (urinalyses) in patient files, utilizing these results to assess treatment success and eligibility for take-home medications.
11. Organize and file various types of patient paperwork into respective charts.
12. Collaborate with medical professionals to facilitate annual justifications, approvals for Step Levels/Exceptions, and dose adjustments for patients.
13. Submit timely requests for take-home medications as deemed appropriate.
14. Document patient progress notes and ensure adherence to counseling frequencies and service units.
15. Conduct patient intake sessions and orient patients to the program.
16. Compose interpretative summaries, transition plans, and discharge summaries for each patient.

8. Maintain presence during working hours at the office to participate in in-person meetings and access necessary computer systems while abiding by company policies.
19. Provide crisis intervention as necessary.

23. Uphold organizational and departmental philosophies, goals, and objectives, while through personal conduct, motivate others to do the same.
24. Communicate pertinent patient information effectively to counselors.

**Qualifications**
- State Certification/Licensure is required, or candidates must be in the process of obtaining the necessary Certification/License. A Certified Alcohol and Drug Counselor (CADC) or Licensed Alcohol and Drug Counselor (LADC) will be accepted.
- Experience working with the opiate-addicted population is required.
- Current certification in CPR and First Aid is preferred.
- Proficiency in conducting assessments, utilizing ASI criteria for suitable patient placement, developing and implementing treatment plans, coordinating services, and executing crisis intervention is required.
- Comprehensive knowledge of local community drug trends, including the impact on bodily and cognitive functions, signs and symptoms of narcotic overdose, psychosocial implications of addictive diseases, the addiction process and treatment, as well as the interplay of social service, economic, legal, and medical systems related to addiction and HIV transmission is essential.
