**Core Job Responsibilities:**
- Attend meetings focused on managing patient care to find ways to improve the treatment provided and suggest solutions when issues arise.
- Help organize monthly training sessions for non-medical staff about important topics related to social work and how it connects to patient care.
- Refer patients to appropriate services and resources.
- Work closely with case managers to create and carry out a program aimed at assisting patients effectively.
- Develop local strategies that bring together the skills of both nursing and social work to enhance patient care.
- Assist in creating and implementing educational programs.
- As a social work case manager, you might also provide additional services that require a licensed clinical social worker.
- Make sure patients are connected to community resources and other government services that can help them.
- Evaluate the emotional and social needs of patients and their families and work with them to create a treatment plan that addresses their issues and strengths, along with the support they need.
- Conduct assessments and provide support to a diverse group of individuals, considering their various backgrounds, including social and economic factors.
- Use knowledge of medical and mental health conditions, treatments, and relevant terminology to assist patients.
**Work Schedule:** Monday to Friday, working 9 hours a day between 7:00 AM and 5:00 PM, with a one-hour lunch break.
**Minimum Qualifications:**

**Education:** A Master’s degree in Social Work (MSW) from an accredited program. -
**Experience:** At least one year of experience in clinical social work or healthcare management within the last five years, working with adults, children, families, seniors, or groups. -
**Licensing:** Must have an active license as a Licensed Clinical Social Worker (LCSW). -
**Certification:** Current Basic Life Support (BLS) certification from either the American Red Cross (ARC) or the American Heart Association (AHA). -
**Security Clearance:** Ability to pass a government background check.
