This contract, on-site role provides a unique opportunity to contribute to the health and well-being of patients facing serious illnesses, ensuring they receive personalized and compassionate care.

As a key member of our medical team, you will play a vital role in delivering palliative care services, collaborating with multidisciplinary teams to develop effective treatment plans tailored to each patient's needs. This position emphasizes a holistic approach to care, aligning with our mission to enhance the quality of life for patients and their families.

 

Responsibilities

  • Provide comprehensive palliative care assessments and recommendations
  • Collaborate with patient families and healthcare teams for holistic treatment
  • Develop individualized care plans that address physical, emotional, and spiritual needs
  • Conduct regular patient evaluations and document care progress
  • Communicate effectively with patients and their families to support decision-making
  • Participate in interdisciplinary team meetings and case discussions
  • Educate patients, families, and staff on palliative care principles
  • Stay current with best practices in palliative care and related fields

Requirements

  • MD or DO degree from an accredited institution
  • Experience in palliative care, geriatrics, or internal medicine is preferred
  • Valid medical license to practice in the state
  • Strong clinical and diagnostic skills
  • Excellent interpersonal and communication abilities
  • Compassionate approach to patient care and understanding of patient needs
  • A commitment to a team-based approach in providing care
  • Ability to build rapport with patients and families effectively

Schedule: Monday-Friday, 8a-5p

Daily Volume: 10 admissions

Patient Population: Geriatric

Duties: Managing chronic conditions, preventative care & assessments, medication management, and care coordination

Support Staff: MA and RN

EMR: Cerner

Must be board-certified

Paid travel and expenses
