- Board Certified in Obstetrics and Gynecology (No Board Eligible Candidates) 

- Required   - Valid Michigan Medical License and Controlled Substances Registration (CSR) at the time of submission  

- Basic Life Support (BLS) Certification - Required  

- Provider must be a local resident

 - REQUIRED, no exceptions  

- Clean background necessary for expedited credentialing - REQUIRED, no exceptions
