**Job Description:**

We are seeking a qualified physician with expertise in geriatrics to conduct comprehensive histories and physical examinations for new participants, along with periodic reassessments based on event-driven indicators. 

The selected candidate will be responsible for formulating individualized medical treatment plans and delivering necessary medical services aligned with the diverse environments where our participants reside.

Key responsibilities include:

- Interviewing, assessing, and evaluating applicants and their caregivers, contributing actively to the Intake and Assessment decisions.

- Engaging in discussions with participants and their caregivers regarding medical conditions and prognoses to facilitate informed decision-making regarding future care strategies.

- Functioning within a collaborative, multidisciplinary culture that emphasizes shared goals across all divisions. - Collaborating closely on nursing treatment programs tailored to individual participant needs.

- As a member of the leadership team, contributing to the establishment and maintenance of strategic operational goals.

- Participating as an integral member of the interdisciplinary team, providing education and insights related to diagnoses, cognitive and physical functions, and various treatment modalities. -

 Leading team and family discussions to promote collective decision-making processes.

- Collaborating with nursing staff to educate participants and their families on health maintenance and chronic disease management.

- Overseeing the execution of the healthcare and treatment program as designed by the interdisciplinary team.

 Conducting physical examinations, ordering, and interpreting relevant laboratory and diagnostic tests, with systematic documentation of results.

- Participating in provider on-call rotations as required by operational needs.

**Qualifications:**

- Current and active plenary physician license to practice in North Carolina.

- Board Certification in Internal Medicine or Family Practice, with an emphasis on Geriatrics preferred.

- Demonstrated experience or additional qualifications in geriatrics.

- Required certifications: CDS Certification and DEA Registration.

- Minimum of one (1) year of documented clinical experience with a frail or elderly population.

- Proficiency in physical assessment skills and chronic disease management specific to geriatric patients.

- A proactive desire to work in an innovative alternate healthcare setting.

- Ability to effectively manage shifting priorities based on participant needs.

**Certification Requirements:**

- Board Certified. - Controlled Substance Registration (CSR/CDS). - DEA Registration.

- State Licensure: North Carolina.

- CDS Certification and DEA Registration are mandatory; experience or additional qualifications in geriatrics are highly desirable.
