**Job Description:**
- You will work under the supervision of the Executive Director.
- You will help to oversee the medical care of participants.
- You are responsible for making sure participants receive good care and that we improve quality through our programs.
- You will help create and maintain medical rules and guidelines.
- You will guide and supervise Primary Care Providers (PCPs) and other medical professionals.
- You will provide leadership and support to team members and local community providers.
- You need to have a driver's license, insurance, and a way to get around.
- You should know how to use Microsoft programs like Word, Excel, PowerPoint, and understand email and calendars.
- You should have good organizational skills, provide excellent customer service, and be flexible in solving problems.
- You should be willing to travel within the region when needed.
- You will do health check-ups for new participants and assess their needs regularly. You will create medical treatment plans and provide necessary medical services wherever the participants are located.
- You will interview and evaluate new participants and their caregivers and help decide if they can join the program.
- You will talk with participants about their health and help them and their caregivers plan future care according to their wishes.
- You will work together with other divisions within TH PACE in a friendly and cooperative way.
- You will cooperate in caring for participants with the nursing team.
- As a part of the leadership team, you will help create a plan to reach operational goals.
**Qualifications:**
- You must be Board Certified in Internal Medicine or Family Medicine – this is required.
- It’s highly preferred that you are also Board Certified in Geriatrics or have additional qualifications in Geriatric Medicine (this should be completed before starting).
- You need to have an Indiana medical license or be willing to get one – this is required.
- A clean malpractice record is highly preferred.
- You must have CDS Certification by the time you start – this is required.
- You will need DEA verbal confirmation before starting – this is required.
- You must have BLS certification by the time you start – this is required.
- You should have at least 1 year of experience working with elderly or frail individuals – this is required.
- Previous experience in a supervisory role is required.
