We are currently seeking a skilled Nurse Practitioner (NP) specializing in Family Medicine . As an NP in Family Medicine, you will play a crucial role in providing comprehensive healthcare to patients of all ages within a family practice setting. Your expertise will be vital in promoting wellness, preventing diseases, and managing chronic conditions, contributing significantly to the overall well-being of our patients.

In this role, you will be involved in conducting physical exams, diagnosing illnesses, developing treatment plans, prescribing medications, and educating patients on maintaining a healthy lifestyle. Your dedication to delivering high-quality, patient-centered care will be instrumental in building strong relationships with individuals and families, ensuring their ongoing health and satisfaction. 

Responsibilities:

  • Performing comprehensive assessments and developing individualized care plans for patients.
  • Ordering and interpreting diagnostic tests to aid in accurate diagnosis and treatment.
  • Prescribing medications and treatments based on evidence-based practice guidelines.
  • Providing patient education on disease prevention, health promotion, and treatment options.
  • Collaborating with physicians and other healthcare professionals to ensure coordinated patient care.
  • Managing chronic conditions such as diabetes, hypertension, and asthma through regular monitoring and follow-up.
  • Assessing acute illnesses and injuries, determining appropriate interventions for timely recovery.
  • Documenting patient encounters and maintaining accurate medical records in compliance with regulations.

Requirements:

  • Master's degree in Nursing with specialization in Family Medicine.
  • Active Nurse Practitioner license in the state of practice.
  • Certification as a Family Nurse Practitioner (FNP) from an accredited program.
  • Demonstrated experience in providing primary care services to patients across the lifespan.
  • Strong clinical assessment and diagnostic skills for accurate patient evaluation.
  • Excellent communication and interpersonal abilities to establish rapport with patients.
  • Proficiency in electronic health record (EHR) systems for documentation and charting.
  • Ability to work collaboratively in a multidisciplinary healthcare team environment.

Schedule: Monday-Friday, 40 hours per week

No call requirement

Patient Volume: 15-18 per day, ~2.5 per clinical hour

EMR: Epic

Experience: 1+ year

Board certification required
