We are currently seeking a compassionate and dedicated Nurse Practitioner (NP) specializing in Hospice and Palliative Care . In this role, you will play a crucial part in providing quality end-of-life care to patients, ensuring their comfort and dignity during challenging times. Responsibilities:
- Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs.
- Develop and implement individualized care plans in collaboration with the interdisciplinary team.
- Manage symptoms such as pain, nausea, and shortness of breath through appropriate interventions.
- Provide education and support to patients and their families on end-of-life care options.
- Coordinate care with other healthcare providers to ensure holistic patient care.
- Document patient care activities accurately and timely in electronic health records.
- Participate in regular team meetings to discuss patient progress and care plans.
- Adhere to all regulatory standards and guidelines related to hospice and palliative care.
Requirements:
- Active Nurse Practitioner (NP) license in the state of practice.
- Certification in Hospice and Palliative Care preferred.
- Strong communication and interpersonal skills.
- Ability to provide compassionate and empathetic care to patients and families.
- Knowledge of pain and symptom management in palliative care.
- Experience working in end-of-life care or hospice settings is a plus.
- Proficiency in electronic health record systems.
- Ability to work collaboratively in a multidisciplinary team environment.
About the Position:
Schedule: Monday-Friday (8a-5p) or 4x 10-hour shifts
No call
Setting: Outpatient
Daily Census: 10-20 patients
Will travel to different homes, facilities, and clinics
APPs practice independently with some supervision
EMR: AthenaHealth
Board certification required
Preferred Experience: 2+ years
