Nurse Practitioner delivering comprehensive, patient-centered primary care to frail and elderly populations within a collaborative and mission-driven environment.
**Job Description:**
- Collaborate closely with organization physicians to provide primary care services.
- Perform thorough assessments of participants, including initial, ongoing, and as-needed evaluations.
- Develop, implement, and manage individualized care plans.
- Conduct evaluations and treatment for acute illnesses.
- Participate in discharge planning for transitions from hospital or long-term care settings.
- Conduct home visits as necessary to ensure continuity of care.
- Assist in the development and implementation of clinical policies and procedures.
- Actively engage in interdisciplinary team (IDT) meetings.
- Deliver care specifically focused on geriatric and frail populations. - Ensure accurate and timely documentation of all clinical encounters.
- Participate in the provider on-call rotation.
- Foster a compassionate and patient-centered care environment.
*Qualifications:** - Board Certified Nurse Practitioner (ANCC or AANP) required.
- Active Nurse Practitioner license is mandatory.
- DEA registration is necessary. - CDS certification is required.
- BLS certification is essential.
- A minimum of 5 to 7 years of Nurse Practitioner experience is required, preferably in long-term care.
- Experience with frail or elderly populations is desirable, although willingness to undergo training is acceptable
. - A clean malpractice and license history is mandatory.
