**Job Description**
**General Responsibilities:**
- Conduct thorough medical assessments to inform patient care.
- Design and execute tailored care plans for individual patients.
- Evaluate and treat acute illnesses in various settings.
- Engage actively in discharge planning to ensure continuity of care.
- Perform home visits as needed for patient assessment and support.
- Facilitate seamless coordination of care across the full spectrum of services provided
- Contribute to the development and review of clinical policies and procedural guidelines.
- Take part in the provider on-call rotation as necessary.
**Qualifications:**
- Board Certified Nurse Practitioner (ANCC or AANP) – **[REQUIRED]
** - Current, active Nurse Practitioner license in Massachusetts –
**[REQUIRED]** - DEA Registration to prescribe controlled substances –
**[REQUIRED]** - Certification in Controlled Substances (CDS) –
**[REQUIRED]** - BLS Certification –
**[REQUIRED]** - Relevant experience working with frail or elderly populations –
**[REQUIRED]** - Willingness and ability to travel within the designated service area –
**[REQUIRED]**
**Certification Requirements:** - AANP - ANCC
- Basic Life Support (BLS) - Controlled Substance Registration (CSR/CDS)
- Drug Enforcement Administration (DEA) registration
**State License Requirements:** - Valid Nurse Practitioner license in Massachusetts
**Additional Licensure Requirements:** - Nurse Practitioner certification with prescriptive authority.
