Location - Findlay, Ohio

**Job Description:**

- **75% Inpatient Palliative Care; 25% Outpatient Palliative Care**
- Provide complete care for patients in the hospital and those visiting the clinic.
- Help patients manage symptoms, chronic illnesses, plan for their care, and transition to hospice care.
- Have important conversations with patients and their families about serious illnesses and care goals.
- Visit patients in the hospital, including those in the ICU (Intensive Care Unit) who need palliative or hospice care.
- Manage outpatient visits through the Oncology department, infusion center, Comprehensive Health Clinic, and Telehealth.
- Work together with other specialists in the hospital (like ICU, Oncology, Pulmonology, and Cardiology) to create care plans.

**Qualifications:**

- Must be a Board Certified Nurse Practitioner - REQUIRED
- Must have an Ohio license (or be willing to get one) - REQUIRED
- DEA license (must be obtained before starting the job) -
- Experience working as a provider in a hospital setting or with palliative care and hospice is needed.
- Clean malpractice and license - REQUIRED

**Certification Requirements:**

- Must be Board Certified

**State License Requirements:**

- Must have an Ohio license

- Need experience as a provider in a hospital setting or with palliative care and hospice.
