The physical therapist is responsible for conducting accurate assessments, diagnosing conditions through differential analysis, and identifying deficits affecting a patient’s ability to engage in movement and perform activities of daily living. 

This professional develops an individualized treatment plan with patient-centered objectives aimed at alleviating pain, restoring function, and preventing disability, while collaborating with individuals to maintain mobility.

**Essential Functions and Responsibilities:**

- Conducts comprehensive patient evaluations by obtaining medical history, performing relevant system reviews, and selecting and administering specific tests and measures to gather critical data.

- Establishes a personalized plan of care and identifies treatment techniques that target the specific impairments revealed in the evaluation, in alignment with physician recommendations.

- Performs ongoing reassessments of the patient’s condition and their response to the prescribed treatment.

- Initiates and oversees the discharge plan starting from the initial visit and continuing throughout the course of treatment.

- Supervises and delegates tasks to ancillary staff and students in accordance with state laws and departmental policies.

- Completes legible, accurate, and clear documentation within the timeframes established by departmental policies and procedures, adhering to accreditation standards and insurance regulations.

- Communicates effectively with patients, families, caregivers, significant others, and members of the healthcare team to enhance the overall benefit of care, utilizing the AIDET framework.

- Assesses the educational needs of patients, family members, caregivers, or significant others, and provides tailored education to address those needs.

- Engages in professional development through participation in educational programs, including in-service meetings and workshops, as well as by reviewing current literature pertinent to the field.

- Actively contributes to quality improvement initiatives. - Ensures the maintenance of a safe environment for patients, visitors, and staff, complying with established safety standards.

- Operates within the scope of practice as delineated by the state’s practice act.

- Performs additional related duties as directed.

**Qualifications:**

- A minimum of two years of physical therapy experience is required.

**State License Requirements:**

- Must possess a valid license to practice in the state of Oklahoma.
