The physical therapist will conduct a thorough assessment, establish differential diagnoses, and identify deficits in a patient’s capacity to move and perform functional activities in their daily lives. The therapist will develop an individualized treatment plan with patient-centered goals aimed at alleviating pain, restoring function, and preventing disability, while collaborating with the patient to maintain mobility.

**Essential Functions and Responsibilities:**

- Conducts patient evaluations through obtaining medical history, performing relevant systems reviews, and selecting and administering specific tests and measures to collect pertinent data.

- Establishes a tailored plan of care, selecting treatment techniques that specifically address the impairments identified during the evaluation and in accordance with physician orders.

- Conducts continuous reassessment of the patient’s progress and responsiveness to treatment interventions.

- Initiates and manages the discharge plan from the first visit, ensuring continuity throughout the treatment process.

- Supervises and delegates responsibilities to auxiliary staff and students, consistent with state laws and departmental policies.

- Completes documentation that is legible, accurate, and clear, adhering to the established timeframe as outlined in departmental policies and procedures, and in compliance with accreditation requirements and insurance regulations.

- Engages in effective communication with patients, families, caregivers, significant others, and members of the healthcare team to maximize the benefits of care, utilizing AIDET principles.

- Assesses the educational needs of patients, families, caregivers, or significant others and provides appropriate education to address those needs. - Participates in professional growth and development by engaging in educational programs, including in-service training, workshops, and reviewing current literature relevant to the scope of practice.

- Actively contributes to quality improvement initiatives.

- Ensures a safe environment for patients, visitors, and staff, complying with recognized safety standards.

- Operates within the scope of practice as defined by the state's practice act.

- Performs other related duties as required and directed.

 

**Qualifications:**

- A minimum of two years of experience as a physical therapist in a long-term care setting is required.

- A valid Texas or compact license is mandatory.

- Texas

- Compact licenses are permitted.
