**Core Responsibilities:**
- Engage comprehensively across all dimensions of the Case Management Program (CMP), ensuring adherence to established standards for case management (CM) care protocols.
- Provide specialized nursing insights throughout the CM process, encompassing assessment, planning, implementation, coordination, and ongoing evaluation. Proactively identify opportunities for CM integration and adaptability within local process frameworks.
- Design and execute targeted strategies for case management across diverse modalities, including inpatient, outpatient, onsite, and telephonic settings.
- Develop and implement supportive tools for case management operations, such as patient identification and assessment instruments, clinical practice guidelines, algorithms, case management software solutions, and community resource databases.
- Facilitate the convergence of CM with utilization management (UM), effectively integrating nursing case management and social work case management processes.
- Maintain collaborative relationships with relevant community agencies and organizations to enhance resource accessibility and patient care.
- Ensure accurate collection and documentation of patient care data to support quality outcomes.
- Formulate comprehensive treatment plans that encompass preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions, ensuring continuity of care aimed at achieving optimal patient wellness.
- Establish and monitor mechanisms for the effective implementation of patient treatment plans, with a focus on follow-up care in ambulatory and community healthcare settings post-discharge.
- Deliver tailored healthcare education to patients and/or caregivers, addressing specific learning needs identified during assessments.
**Work Schedule:**
- Monday to Friday, 8.5 hours daily within the timeframe of 7:00 AM to 5:30 PM.
- No weekend or holiday commitments.
- No on-call or callback responsibilities.
**Minimum Qualifications:**
- **Degree:** At least an Associate’s Degree in Nursing.
- **Education:** Must be a graduate from a nursing program accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE). Alternatively, a graduate of a State Board of Nursing or an accredited nursing program may be considered.
- **Certification:** Must hold one of the following certifications:
- Certified Case Manager (CCM) from the Commission for Case Manager Certification.
- Certified Disability Management Specialist (CDMS) from the Certification of Disability Management Specialists Commission.
- Certified Rehabilitation Registered Nurse (CRRN) from the Association of Rehabilitation Nurses.
- Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist (COHN-S) from the American Board for Occupational Health Nurses.
- Advanced Certification in Continuity of Care (ACCC) from the National Board for Certification in Continuity of Care.
- Certified Rehabilitation Counselor (CRC) from the Commission on Rehabilitation Counselor Certification.
- Nurse Case Manager (RN-NCM) from the American Nurses Credentialing Center.
- Care Manager Certified (CMC) from the National Academy of Certified Care Managers.
- **Experience:** Minimum one year of nursing experience post-graduation.
- **Licensure:** Must possess an active, unrestricted license to practice as a Registered Nurse in any state.
- **Certification:** Current Basic Life Support (BLS) Healthcare Provider certification from AHA or ARC.
- **Security Clearance:** Must be able to successfully pass a government background check/security clearance.
