**Core Duties:**

  • Engage in all phases of the Case Management Program (CMP) while ensuring adherence to established case management (CM) standards of care.
  • Provide nursing expertise pertaining to the CM process, which encompasses assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and integrate local CM processes effectively.
  • Develop and implement local strategies utilizing inpatient, outpatient, onsite, and telephonic CM services.
  • Create and deploy tools that facilitate case management, including those designed for patient identification and assessment, clinical practice guidelines, algorithms, CM software, and databases relevant to community resources.
  • Integrate case management with utilization management (UM) and combine nursing case management with social work case management practices.
  • Maintain productive relationships with relevant community agencies and organizations.
  • Collect and document patient care data accurately.
  • Formulate treatment plans that encompass preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care with the objective of achieving optimal wellness.
  • Establish mechanisms to guarantee the proper implementation of patient treatment plans and conduct follow-up procedures post-discharge within ambulatory and community healthcare settings. Provide appropriate healthcare education to patients and/or caregivers based on identified learning needs.

**Schedule:**

Monday to Friday; 8-hour shifts between 6:00 AM and 6:00 PM.
No work required on weekends or holidays.

**Minimum Qualifications:**

  • **Degree:** Possess a Bachelor’s degree in Nursing from an accredited university, complemented by one of the certifications listed below; OR possess a Master’s degree in Nurse Case Management from a program accredited by the American Nurses Credentialing Center (ANCC).
  • **Education:** Graduate from a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
  • **Experience:** A minimum of five years of experience as a Registered Nurse Case Manager (RNCM) within the previous seven years is required.
  • **Certification:** Hold and maintain a current certification from one of the following accredited organizations: Certified Case Manager (CCM), Certified Disability Management Specialist (CDMS), Certified Registered Rehabilitation Nurse (CRRN), Certified Occupational Health Nurse (COHN), Association of Community Cancer Centers (ACCC), Certified Rehabilitation Counselor (CRC), Registered Nurse - Nurse Case Manager (RN-NCM), or Certified Management Consultant (CMC); OR possess a Master’s degree in Nurse Case Management from a program accredited by the American Nurses Credentialing Center (ANCC).
  • **Licensure:** Maintain a current, full, active, and unrestricted license as a Registered Nurse.
  • **Security:** The ability to successfully pass a government background check/security clearance is required.
  • **Life Support Certification:** Possess a current Basic Life Support (BLS) Healthcare Provider certification from either the American Heart Association (AHA) or the American Red Cross (ARC).
