**Position Summary:** 

This job involves helping people manage their health care needs while they are at the hospital or clinic. We assist service members, veterans, their families, and caregivers in finding the best care options for their health issues. These issues can include serious illnesses, mental health challenges, or adjusting to life after trauma. We work with many health care providers to ensure that patients receive the care they need. We also consider both medical and emotional support for patients and their families. Evaluating care and outcomes is also part of the job, ensuring that services are provided in a timely manner.

**Core Duties:**

- Be involved in all parts of the Case Management Program (CMP) to ensure it meets the required care standards.

- Use nursing knowledge to help with the case management process, including assessing needs, planning, coordinating care, and monitoring progress. Identify ways to improve our case management and include local processes.

- Create and use plans that include in-person, over-the-phone, and online case management.

- Develop tools that assist with case management, such as methods for assessing patient needs and clinical guidelines.

- Combine case management with other management processes, like those done by social workers.

- Keep in contact with local community agencies and accurately record patient information.

- Create treatment plans that include preventive, therapeutic, and support measures to ensure patients achieve their best health.

- Set up ways to make sure treatment plans are followed after patients leave the hospital or clinic.

- Teach patients and caregivers about health care based on what they need to learn.

**Work Schedule:** - Monday to Friday, working 9 hours between 7:00 a.m. and 5:30 p.m., with a 1-hour lunch break. - Specific hours may vary by department. - No work on weekends or holidays.

**Minimum Qualifications:**

- **Education:** At least an Associate's Degree in Nursing (ASN); a Bachelor's Degree in Nursing (BSN) is preferred for Case Management.

- **Experience:** At least 3 years of experience for ASN or 2 years for BSN in clinical case management with adults, children, and families.

- **Licensure:** Must have a valid and active Registered Nurse license from any state.

- **Clinical Certification:** Must be a Certified Case Manager or eligible to become certified with the required experience in clinical case management.

- **Life Support Certification:** Must have current Basic Life Support (BLS) certification from AHA or ARC. - **Security:** Must be able to pass a government background check.

**Additional Skills Needed:** - Must be able to apply core case management functions effectively, such as:

   - **Assessment:** Identifying patients who need case management and collecting needed information about their health.  - 

**Planning:** Working with patients, families, and health care teams to create a care plan.  -

 **Facilitation:** Coordinating care and communication among everyone involved.  -

 **Advocacy:** Supporting patients and families to ensure they receive proper education and timely care. - Knowledge of medical privacy laws (HIPAA) and health care accreditation standards is important.

- Must be skilled in using computer programs like Microsoft Office and email.

- Able to communicate well with people who may be unwell or upset.

- Must be organized, able to solve problems, and communicate medical information to patients and families with care and respect.
