**Job Description:**
- Provides care to patients based on their needs using the nursing process.
- Keeps up with the rules and standards of professional nursing.
- Supervises Certified Nursing Assistants (CNA) and works with other healthcare workers as needed.
**Responsibilities:**
- Starts with a complete assessment of every patient and checks their health again during follow-up visits. This includes looking at their physical, emotional, and social needs, as well as the resources in their environment and community.
- Creates a Plan of Care for each patient and adjusts it as necessary. Works with the patient, doctors, and other team members to set up plans for when the patient is ready to leave care.
- Completes necessary reports and data collection (OASIS).
- Follows rules set by federal, state, and agency guidelines while providing care.
- Identifies the educational needs of patients and their families, and teaches them as needed.
- Fills out paperwork according to rules and regulations.
- Ensures a clean and safe care environment by following safety practices.
- Respects the privacy and confidentiality of patients and colleagues by following the Standards of Conduct.
**Qualifications:**
- Home Health experience is required.
- Must have a valid driver’s license and car insurance.
- A copy of your Illinois nursing license must be included with your application.
- We will also consider candidates with strong medical-surgical experience along with some home health experience.
**Certification Requirements:**
- Basic Life Support (BLS) certification.
**State License Requirements:**
- Must have a nursing license from Illinois.
