The individual effectively screens, evaluates, and rehabilitates patients with communication, cognitive, or swallowing disorders, and develops an appropriate plan of care in compliance with all regulatory and clinical practice standards. 

Utilizing standardized assessments and evidence-based practices, the professional supports clinical interventions. 

An ongoing assessment of patient progress is conducted, with treatments adjusted accordingly in the clinical record to meet regulatory and clinical practice requirements. 

Treatment outcomes are systematically evaluated for effectiveness, and services are modified based on patient evaluations; referrals are made as necessary in relation to observed outcomes. 

Consultation and counseling are provided to patients, as well as to their families, caregivers, and other service providers concerning speech disorders.

Effective communication is maintained with supervisors and other health team members regarding patient progress, barriers encountered, and treatment plans.

 A comprehensive discharge summary of services provided is prepared in accordance with regulatory and clinical requirements. 

All regulatory and clinically required information is documented in the electronic documentation system, with patient records maintained in alignment with company policy. 

Treatment minutes for services provided are accurately recorded to meet all regulatory and clinical requirements.

The individual actively participates in requisite meetings, including but not limited to patient care conferences, utilization review meetings, and rehabilitation meetings. Participation in facility and company-mandated training sessions, in-services, and conferences is also required.

 

The professional is proficient in obtaining any necessary durable medical equipment (DME) or supplies in accordance with regulatory and clinical practice requirements and is capable of supervising and supporting Clinical Fellowship Year (CFY) or Student Speech-Language Pathologists.

 

Qualifications include a minimum of one year of experience as a Speech-Language Pathologist (SLP) which is mandatory. 

While new graduates are acceptable, they must possess Level II experience rather than merely Level I observational experience. 

Experience in Long-Term Care (LTC) or Skilled Nursing Facilities (SNF) is essential. 

A Missouri State License is required, and Certification of Clinical Competence (CCC) is mandatory. 

Preference will be given to candidates with three years of experience holding active Clinical Competence Standards (CCS).
