Job Title: Senior Full Stack Engineer – Healthcare Payers & HIE Special Projects (CTOs Office)
Location: Remote
Working Hours: 3 PM IST to 12 AM IST
Experience Required: Minimum 7 years
Employment Type: Full-Time
Department: CTO Office / Special Projects (Healthcare Engineering)

Position Overview
The Senior Full Stack Engineer in the CTO Office will own and deliver high-impact special projects across our healthcare portfolio—primarily focused on health insurance payers and Health Information Exchange (HIE) initiatives. This is a high-autonomy, high-visibility role for someone who thrives in ambiguity, moves fast with strong engineering judgment, and can independently take a project from concept to production. You will partner directly with the CTO and cross-functional stakeholders to identify opportunities, shape solutions, build end-to-end systems, and drive adoption—balancing speed with quality, security, and interoperability requirements in healthcare.

Key Responsibilities

  • Own special projects end-to-end: discovery, solution design, implementation, testing, deployment, and post-launch iteration.

  • Partner directly with C-level execs to translate strategic priorities into executable technical plans, milestones, and shipped outcomes.

  • Design and build production-grade full stack applications using Next.js/React, Node.js, and TypeScript.

  • Architect scalable, secure systems that handle PHI responsibly, including audit logging, traceability, access controls, and data minimization.

  • Build backend services and APIs (REST and/or GraphQL) with strong standards for reliability, documentation, versioning, and performance.

  • Lead integration efforts for payer and/or HIE workflows, enabling data exchange and interoperability across systems.

  • Implement or support healthcare data exchange standards and transactions such as FHIR, HL7 v2, and X12/EDI (270/271, 276/277, 278, 834, 835, 837) as project needs require.

  • Make pragmatic architecture decisions and technology selections, documenting tradeoffs and ensuring maintainability.

  • Establish best practices for the project: code quality, automated testing, CI/CD, monitoring, and operational readiness.

  • Work closely with product, operations, compliance, and engineering teams to align requirements, unblock execution, and ensure successful rollout.

  • Communicate proactively with stakeholders—providing clear status updates, risks, options, and recommendations.

Required Skills and Qualifications

  • Bachelor’s degree in Computer Science, Software Engineering, or a related field.

  • 7+ years of full stack engineering experience shipping production applications.

  • Strong expertise in Next.js/React and modern front-end development (state management, performance, accessibility).

  • Strong backend experience with Node.js and TypeScript (Express/NestJS or similar), including API design and integration patterns.

  • Proven ability to work independently and deliver projects from start to finish with minimal oversight.

  • Strong system design skills and experience building scalable architectures, making sound tradeoffs, and driving projects through ambiguity.

  • Solid experience with SQL databases (Postgres/MySQL) and data modeling; familiarity with NoSQL is a plus.

  • Healthcare domain experience, preferably with health insurance payers and/or HIE programs.

  • Working knowledge of interoperability standards and workflows (FHIR, HL7, X12/EDI).

  • Experience building secure systems in compliance-driven environments (HIPAA-aligned controls, encryption, least privilege, auditability).

  • Experience with cloud platforms (AWS/Azure/GCP), CI/CD pipelines, and production deployments.

  • Excellent written and verbal communication skills, including the ability to align technical execution with business outcomes.

Preferred Qualifications

  • Hands-on experience with payer workflows such as eligibility & benefits, claims lifecycle support, prior authorization, provider data, member enrollment, care coordination, and payer reporting.

  • Experience with FHIR implementation details (profiling/validation, terminology, subscriptions, bulk data, integration engines).

  • Experience with HIE integration patterns and tooling (interface engines, exchange workflows, eventing, batch vs real-time pipelines).

  • Experience with observability (logs/metrics/traces), incident response, and building reliable production systems.

  • Exposure to event-driven architectures (queues/streams), background processing, and resilient integration patterns.

  • Familiarity with compliance frameworks and operational rigor (SOC 2 environments, HITRUST-aligned controls).

  • Experience with infrastructure-as-code and containerized deployments (Docker/Kubernetes/serverless).

Soft Skills

  • Highly self-driven with strong ownership—able to take initiative, make progress independently, and deliver outcomes.

  • Comfortable operating in ambiguity with a builder mindset—able to define the problem, propose solutions, and execute quickly.

  • Strong product thinking—understands user needs and optimizes for impact, usability, and adoption.

  • Excellent stakeholder management—clear, proactive communication and ability to influence without authority.

  • High standards for quality, security, and reliability—especially when dealing with sensitive healthcare data.

  • Resilient and adaptable—able to pivot priorities while maintaining focus and execution discipline.

What Makes This Role Different

  • Direct partnership with C-level execs  and high visibility across strategic initiatives.

  • High autonomy and ownership—your work will directly shape product direction and execution.

  • A chance to build meaningful healthcare technology that impacts real patients, providers, and payer operations.

  • Opportunity to set the engineering bar for special projects through strong architecture, speed, and craftsmanship.
